Making your appointment
Please have your insurance information and referring doctor's information ready when making an appointment with us. This gives our staff adequate time to obtain any referrals necessary and to verify the details of your insurance plan.
We are able to accommodate most urgent appointments that are referred by a physician. Appointments are usually available within a 1-2 week period. We offer convenient early morning and evening appointments.
If you need to cancel or reschedule an appointment, kindly inform us at least 24 hours prior to your office visit so we can give that slot to a patient who may need to see us urgently. Please note there is a fee of $50 for cancellation less than 24 hours or if you fail to show for your appointment.
If you are scheduling an EMG (electromyography) or EEG, please be advised that there is a $100 fee if the appointment is not canceled with at least 24 hours notice.
It is your responsibility to assure that all necessary referrals have been requested from your primary care provider in advance. If we do not have a referral on file 48 hours prior to the appointment, we reserve the right to cancel your appointment and reschedule after the referral has been received.
High deductible plans and Self-Pay patients
To secure your appointment slot, we require a pre-payment by credit card of $50. The card will be charged the day prior to the visit and will apply to the visit. If you fail to show for the appointment or have a cancellation less that 24 hours before the visit, the $50 dollar prepayment will be converted to a no-show fee. We offer payment plans if needed, please inquire about this prior to your appointment check-in.
To speed up your intake process during your visit, please download, print, and fill out the new patient forms at home and bring them to your appointment.
